What’s Home Help Program?
Home Help Program is a Medicaid Program run by Centre HRW that primarily serves the elderly and disabled who require assistance with daily and basic living tasks. Personal care services, such as toileting and bathing, are provided through this program, as well as aid with washing and shopping for groceries. The goal of the Home Help Program is to encourage independence at home while delaying institutionalization. As a result, it is not provided to people residing in assisted living residences, adult foster care homes, or nursing homes.

What Does This Program Include?
This program includes a consumer direction component, often known as self-direction or participant direction. Participants in the program can choose their own caregivers, according to the Centre HRW model. Family members and friends (excluding spouses and parents) can be compensated for the care they offer.

When Do You Need This?
When care is required at home or near to home, primary health care service providers from Centre HRW’s employees and volunteers are your best shot. We are frequently your initial point of contact with the health-care system. It is our responsibility to assist you in resolving your current health issues. And, if you require more expert assistance, we will show you how to navigate the health-care system.

Benefits and Services:
The services of this program are arranged on a case-by-case basis with a county worker. The most common services provided are assistance with activities of daily living and instrumental activities of daily living. These are some of the activities:
